The Lutheran Health Network Mobile Integrated Health Program offers post-hospitalization follow-up and assessment provided by specially trained Lutheran EMS paramedics. The goal of the program is to reduce readmissions and address immediate health concerns for patients at a higher risk for post-hospitalization complications.
